HEALTH REQUIREMENTS FOR ENTERING PRE-KINDERGARTEN*
Deadline for the following requirements is the second Monday of September. If this required information or appointment card is not turned in to the school/nurse, your child will be excluded from school.
1. School Physical
Done within 1 year prior to initial pre-kindergarten year

2. Immunization requirements:
*If your child does not receive immunizations for religious reasons, the Religious Exemption form must be completed by a parent/guardian and your child's health care provider. Refer to the link below for the Religious Exemption form.
*If your child does not receive immunizations for medical reasons, please attach a note from the physician stating reason(s) why immunizations are not given.
3. Turn in all health information (physical and immunization record) to your child's school.

2. Immunizations: Meningococcal, TDap booster and Varicella
***Fall 2015/2016 school year Meningococcal Conjugate Vaccine will be a requirement for 6th grade***
3. Dental Exam
Dental exams are to be completed within 18 months of the May 15th deadline. Dental exams or appointment cards can be turned into the school throughout the school year. Dental exam forms are available at the dentist office, from the school nurse, or Click Here to download.
1. There are no specific health requirements for 7th and 8th grade.
2. If your child will be trying out for any sport, they must have an athletic physical on file. The athletic physical is good for one year from the date of the physical. After this date, another physical must be obtained for the student to continue participating.
3. Athletic physical forms are available at the doctor's office. School physical forms may also be used for junior high sports. School physical forms are available at the doctor's office, from the school nurse, or see link below for physical form.
Vision and hearing screenings are on-going throughout the school year. If the parent/guardian does not want their student screened please put in writing and send to your student's nurse. (This serves as notice per IDPH request.)
