EFFECTIVE SCHOOL YEAR 2016-2017: WE ARE UNABLE TO EXCEPT APPOINTMENT CARDS AFTER OCTOBER 15TH. TO AVOID EXCLUSION PHYSICAL AND IMMUNIZATIONS MUST BE TURNED IN BY OCTOBER 15TH.

HEALTH REQUIREMENTS FOR ENTERING PRE-KINDERGARTEN*
Deadline for the following requirements is the second Monday of September. If this required information or appointment card is not turned in to the school/nurse, your child will be excluded from school.


1. School Physical

Done within 1 year prior to initial pre-kindergarten year

2. Immunization requirements:

  • DTP/DtaP or Td (diptheria, pertussis, tetanus) - 4 doses (1 dose after 2nd birthday)
  • Polio (IPV and/or OPV) - 3 doses (1 dose after 2nd birthday)
  • MMR (measles, mumps, rubella) - 1 dose ( after 1st birthday but before 2nd birthday)
  • Hib (haemophilus influenza type b) - 1 dose
  • Pneumonia - 4 doses
  • Hepatitis B Series - 3 doses given at proper intervals
  • Varicella (chicken pox vaccine) - 1 dose or documentation of chicken pox by physician (after 1st birthday)

*If your child does not receive immunizations for religious reasons, the Religious Exemption form must be completed by a parent/guardian and your child's health care provider. Refer to the link below for the Religious Exemption form.

*If your child does not receive immunizations for medical reasons, please attach a note from the physician stating reason(s) why immunizations are not given.

3. Turn in all health information (physical and immunization record) to your child's school.

Other Health Requirements:

1. Dental examination done within 18 months of the May 15th deadline or appointment card needs to be turned into the school office. Forms available at the dentist office, from the school nurse, or Click Here to download.

2. Vision examination is required to be completed and turned into the school by October 15th. Forms available at the optometrist's office, from the school nurse, or Click Here to download.


2ND GRADE HEALTH REQUIREMENTS


1. Dental examination done within 18 months of the May 15th deadline. Dental exams or appointment card for after May 15th can be turned into the school anytime during the school year. Dental exam forms are available at the dentist office, from the school nurse, or Click Here to download.


6TH GRADE HEALTH REQUIREMENTS

Deadline for the following requirements is the second Monday in September. If this information is not turned into the school nurse your child will be excluded from school.

1. School Physical: Done within 1 year of starting 6th grade.


2. Immunizations: Meningococcal, TDap booster and Varicella

***Fall 2015/2016 school year Meningococcal Conjugate Vaccine will be a requirement for 6th grade***

  • DTP/DtaP or Td (diptheria, pertussis, tetanus) - 4 or more doses with the last dose received on or after 4th birthday Polio (IPV and/or OPV) - 3 or more doses with the last dose received on or after the 4th birthday
  • MMR (measles, mumps, rubella) - 2 doses at least 1 month apart Varicella (chicken pox vaccine) - 2 dose or documentation of chicken pox by physician
  • Hepatits B- 3 doses
  • Meningococcal- 1 dose

3. Dental Exam

Dental exams are to be completed within 18 months of the May 15th deadline. Dental exams or appointment cards can be turned into the school throughout the school year. Dental exam forms are available at the dentist office, from the school nurse, or Click Here to download.


7TH & 8TH GRADE HEALTH REQUIREMENTS

1. There are no specific health requirements for 7th and 8th grade.

2. If your child will be trying out for any sport, they must have an athletic physical on file. The athletic physical is good for one year from the date of the physical. After this date, another physical must be obtained for the student to continue participating.

3. Athletic physical forms are available at the doctor's office. School physical forms may also be used for junior high sports. School physical forms are available at the doctor's office, from the school nurse, or see link below for physical form.
